Hazardous-waste sites cited for rule violations

The state Department of Health has cited two local hazardous-waste generators for violating hazardous-waste rules, officials said yesterday. The two face possible penalties of more than $30,000 each.

DOH cited Honolulu Shipyard Inc. for improperly treating and handling paints and solvents, operating a hazardous-waste treatment facility without a permit and mismanaging hazardous containers. The company faces a possible $45,000 penalty.

Hawaiian Steam Inc. was cited for improperly transporting oil, operating without a used-oil processor permit and not tracking used oil received from generators. The company faces a possible $33,700 penalty.

Both companies have requested hearings to contest the allegations and penalties.

HECO seeks cause of Thursday outages

Hawaiian Electric Co. officials have yet to find the cause of a series of power failures Thursday afternoon which affected an estimated 30,000 to 40,000 customers, including two hospitals.

The power failures forced Kaiser Medical Center and Wahiawa General Hospital to go on emergency generators and sent police officers to direct traffic at major intersections.

The outages were caused when three Oahu generators went offline just before 3 p.m., triggering a protection system that cuts off power to customers to prevent an islandwide failure, according to HECO spokesman Fred Kobashikawa.

HECO personnel still had not identified yesterday what caused the three generators to go down and how a similar massive outage could be prevented, Kobashikawa said.

"It's something that requires investigation. There is a lot of engineering data that has to be reviewed."

Stolen car set ablaze visible from Tantalus

Honolulu fire officials said a stolen car that was set on fire started a blaze at the 3000 block of Tantalus which could be seen by motorists on the H-1 freeway.

Man stabbed aboard longline fishing vessel

FBI, INS and U.S. Coast Guard investigators are expected to meet the crew of the longline fishing vessel Pacific Horizon, where a stabbing took place 125 nautical miles east of Maui yesterday afternoon.

A Coast Guard spokesman said the incident was reported at 2 p.m., and a helicopter arrived at the scene about an hour and 45 minutes later.

The Coast Guard airlifted the victim, described as a 30-year-old male Filipino national, and transported him to Maui for treatment.

Coast Guard officials said the suspect who allegedly stabbed the victim was separated from the rest of the crew.

There are nine members in the crew. There was apparently some sort of argument that escalated, leading to the stabbing, an official said.

The suspect was only identified as being a Filipino national.
