MDA achievement nominations sought
The Muscular Dystrophy Association is seeking nominations for its 14th annual Personal Achievement Awards. The award recognizes the accomplishments of people with disabilities in Hawaii caused by neuromuscular diseases.
Nomination forms are available from the MDA office and must be submitted by Jan. 23. Individuals or organizations may make nominations. The only qualification is that the nominee be affected by one of the 43 neuromuscular diseases in MDA's program and be registered with this association.
MDA started the awards program in 1992 to honor the contributions of people with disabilities in their local communities. Since then, MDA has recognized more than 1,000 award recipients. Award recipients are invited to appear on locally broadcast segments of the Jerry Lewis MDA Telethon.
For nomination forms, call 593-4454 or e-mail jli@mdausa.org.
