Konishiki withdrawsMusashimaru, or Fiamalu Penitani, kept a share of the lead at 4-0 but Konishiki, or Salevaa Atisanoe, was forced to withdraw due to persistent swelling in his left calf muscle.
Musashimaru, who chased Kaio around and out of the ring, is tied for first place with fellow champion Wakanohana, who was awarded a judges' decision over previously unbeaten Dejima after they fell off the raised dohyo together.
Akebono and fellow grand champion Takanohana are in a group at 3-1.
Konishiki, who had lost his first three bouts, withdrew after being told by doctors at a Tokyo hospital that he would need about two weeks of treatment to recover.
"I took painkillers this morning but the pain wouldn't go away. It would be dangerous if I hurt my leg even more," Konishiki said. "Two, three days later I'll see how things are and I intend somehow to reenter the competition."
Konishiki has suffered repeated calf injuries during his long sumo career and Wednesday's withdrawal is his 11th from a grand sumo championship.
