ReplaceMedicare card at
Social Security office

QUESTION: I lost my Medicare card. How do I go about getting a replacement?

ANSWER: You can have it replaced by applying in person at your local Social Security office or by calling (800) 772-1213. It will help speed the replacement procedure if you can tell Social Security representatives your Medicare number. If you can't, they will be able to track it down through your Social Security number.
